All organizations seeking a donation from Lambert’s Rainbow Fruit must download and print the form below.

• Fill out the entire form
• Bring the form into a local Lambert’s Store and speak to the manager
• Your application will be reviewed
• Please bring all applications in at least 30 days before your event.
